We are transitioning to electronically processing immigration benefit requests. As we complete this transition, we will be using multiple systems to receive and process various types of immigration benefit requests. Because the Form I-129 and related applications are not all processed in the same system, we require a separate payment instrument for each of these forms.

We acknowledge that using multiple checks or payments for the Form I-129 and related applications is more burdensome than using one payment. We are always analyzing our forms, practices, and policies to streamline filing and reduce, minimize, and control burdens to submit requests. However, we believe that the advantages of electronic processing to both the agency and to the public outweigh the minor inconvenience of submitting individual fee payments.
We encourage you to apply for an SSN (or replacement SSN card) using Form I-765, Application for Employment Authorization, and following the form instructions. If you do not request an SSN in Part 2 (Items 14-17.b) of your Form I-765, you must make an appointment to visit a Social Security Administration office in person to apply for your SSN after you receive your Employment Authorization Document (Form I-766). There is no filing fee or biometric services fee for the initial Form I-765 filed by current or former service members who were paroled into the United States under the Immigrant Military Members and Veterans Initiative (IMMVI). Please write "IMMVI" at the top of Form I-765 to obtain the fee exemption and submit documentation that supports current or former military service such as the Certificate of Release or Discharge from Active Duty (DD Form 214), National Guard Report of Separation and Record of Service (NGB Form 22), or other official service or discharge document.
Pay each filing fee separately. We are transitioning to electronically processing immigration benefit requests, which requires us to use multiple systems to process your package. Because of this, you must pay each filing fee separately for any form you submit. We may reject your entire package if you submit a single, combined payment for multiple forms.
Use Form I-9 to verify the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must properly complete Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. Both employees and employers (or authorized representatives of the employer) must complete the form.

Only employers and employees in Puerto Rico can complete the Spanish version of Form I-9. Spanish-speaking employers and employees in the 50 states and other U.S. territories may print this for their reference, but must complete the form in English to meet employment eligibility verification requirements.

You should only submit the forms listed below for special situations. 1. Reporting Your Passport Lost or Stolen (DS-64)You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.
Implementing forms is typically a short process. Because there is no data migration or background work needed, the implementation time is only as long as it takes you to create the form and its fields.
Controls the annotations and what kinds of links the form creates. Annotations include external, nofollow, opener, noopener, and noreferrer. Link types include help, prev, next, search, and license. The rel value is a space-separated list of these enumerated values.

Please note that these forms are not designed for printing on standard 8.5 x 11" paper. If you want to print on 8.5 x 11" paper, you may need to change your Acrobat application settings to use "Shrink to Fit", and you may also need to make configuration changes for your specific printer. It is also recommended that you do not attempt to print these forms from your web browser -- download the PDF file and open it directly in Acrobat.
If you do not have the ability to reproduce the original 8.5 x 14" forms, you may want to contact a local printer or publishing company to have the forms professionally reproduced for you -- simply provide the printer with a copy of the PDF file on disk.
Records are accessioned into the National Archives, and become archival, 62 years after the service member's separation from the military. This is a rolling date, the current year minus 62 years. Please note: After December 12, 2015, MCS-150 forms can ONLY be used to update a USDOT number, not for initial registration with FMCSA. First-time applicants must use the Unified Registration System (URS). For more information about URS, or to apply for the first time, click here.
These instructions assume that the applicant has determined that a USDOT Number is required to update the US DOT number registration and that the MCS-150 form must be submitted. Again, this form can only be used to update existing registrations as of December 12, 2015.
